Free LASIK evaluation is Free for eyes which are normal, have no history of any previous surgery (Radial Keratotomy- RK, Cataract surgery, Lasik surgery, Laser Vision Surgery etc.) or any pathology. Since we utilize advanced diagnostic technologies and a detailed examination protocol at our institute, we may during your evaluation, diagnose or discover pathologies in your eyes like cataracts, glaucoma, Macular Degeneration-AMD, high astigmatism, keratoconus etc which may differ the choic eof Lasik surgery as your optimal care. Having diagnosed any such variant element or pathology, we will as per our mission to deliver the best care for your eyes, suggest appropriate medical attention to the same.

Privacy Policy

At the Gulani Vision Institute, it is important to us, that you understand your rights according to the Health Insurance Portability and Accountability Act (HIPAA). The responsibility of Gulani Vision Institute is to guard your privacy. It is our duty to maintain the privacy of your Protected Health Information (PHI). PHI (Protected Health Information) may be used to carry out treatment, payment or health care operations in our practice. Your health information may be used to help treat your disease or problem and it may be disseminated to other health care providers in your best interest to help provide you with excellent health care. PHI may also be used in communicating with other doctors who are taking care of you, insurance companies for payment, and for our internal operations. Gulani Vision Institute will also send post-card reminders for appointment dates; may contact you by phone; may leave voice messages; and send out e-mails. All other uses and disclosures will be made only with your approval.

Our patients have the right to request restrictions on uses and disclosure of PHI (Protected Health Information) for treatment, payment, and healthcare operation purposes. Should our patients choose to revoke this consent, it must be given in writing to the practice.

You have the right to see your own designated record set and may make such a request in writing to our office. You have the right to file complaints against our office if you feel your privacy has been violated. Please report any such violations to the Gulani Vision Institute at (904)-296-7393. You also have the right to report any violations to the Department of Health and Human Services. This notice is effective April 1, 2004.
